Appointment policy:
Your child’s health is of utmost importance. Therefore a parent must be present for all well child and med check visits. If a parent is unable to bring their child in for a sick visit, an authorized adult is allowed to bring the child with a note from the parent.
Well child visits (aka check-ups, annual physicals and sports physicals):
Your visit can be scheduled with the physician of your choice.
All children, even if they are healthy, should have an annual check-up (babies need to be seen even more often). Check-ups allow us to monitor your child's growth and development, screen for disease, and administer necessary vaccinations.
Sick child visits:
We offer same day, first available appointment with first available physician.
No Show/Cancellation Policy:
You may cancel an appointment with no charge any time before 7pm on the day preceding your appointment. Same day cancellations or a “no show” for your scheduled appointment will result in a $25 charge for a sick visit and $50 charge for a well child exam.
Please note that if you continue to miss your child’s appointment, we may ask that you establish with another office.
After-hours procedures:
If you feel that your child’s problem after-hours may be emergent then call 911 or go to the nearest emergency room. We recommend St. John-Macomb Township Urgent Care or Pediatric After Hours Clinic - Troy Beaumont for your after hours needs. If you need to speak to the physician on call, please call 586-558-2111 and the on-call physician will be contacted.
We recommend using Pediatric Emergency Departments when possible. Recommended Pediatric Emergency Departments include Ascension St. John Hospital in Detroit, Beaumont Hospitals in Royal Oak or Troy, Children’s Hospital of Michigan in Detroit or Troy. Recommended Pediatric Urgent Care Centers include the Pediatric After-Hours Clinic at Troy Beaumont Hospital or Troy Pediatric Urgent Care. Our answering service can be reached at 844-538-1834 if you need urgent medical advice when the office is closed.
Patient forms:
Forms (physical, camp, sports, new patient) can be downloaded from our Patient Forms page – Parent sections of the form need to be completed before physicians can complete our sections. Sports physical forms will be completed if appropriate for your child and if they have had a checkup in our office in the past year. There is a $20 fee for completing some patient foms.
**Note: If your child has had Covid and plans on participating in a sport, they will be required to be seen prior to completing the sport’s physical form**
